Every second Tuesday in May, we mark ‘Stroke Day’ in Europe. On this day, attention is drawn to recognising a stroke (CVA) and what to do when you or someone close to you suddenly has one of the symptoms of a stroke. But what if you have (had) a stroke yourself? Then we, the CVA Care Chain West-Brabant and Tholen, are here for you.
The CVA Care Chain West Brabant and Tholen is a collaborative project of different organisations (such as home care organisations, physiotherapists, (geriatric) rehabilitation centres, and a hospital). As a chain, we are involved in the care of people who have had a stroke. Our intensive cooperation ensures that the lines between the organisations are short, that we can switch quickly and that we understand each other well. This allows us to constantly develop, adjust and improve the care for our patients. We really do this together and we are proud of it! To make our cooperation even more transparent, a booklet has been developed. In this booklet, each organisation tells something about themselves and the care they provide to clients. We also share some stories of our own clients. These stories give a picture of the (care) path they went through after their stroke and how they experienced it.
